Creating healthcare SaaS is not a simple engineering process. The company you partner with to create your application should have experience in handling PHI, meeting healthcare security needs, working with EHR systems, and developing apps that can pass compliance audits. Otherwise, you risk spending additional money on reworking your app and fixing security flaws, as well as delays in its launch.
When choosing a healthcare software development company, organizations need to consider other aspects apart from general software development. These include healthcare industry knowledge, experience with PHI, understanding of regulatory requirements, EHR integration skills, having the necessary security certifications, such as HITRUST, and a portfolio of healthcare solutions.
Cost of development, technical skills, and client testimonials may be some of the other factors that help decide whether the company will be able to achieve the desired specifications of the product. Based on this, we evaluated the below-mentioned companies, considering their specialties, certification, and estimated cost of development.
Essential Parameters for Evaluating Healthcare SaaS Development Companies
Analyzed the stats for healthcare SaaS and software development companies from Clutch, GoodFirms, and G2 and then narrowed them down in four steps.
- Healthcare track record: We assessed each company's publicly available healthcare portfolio, including published project descriptions, case studies, and client reviews covering telemedicine, EHR systems, patient applications, and AI-enabled clinical tools.
- Security & compliance: We examined all the company's available certifications (HITRUST, SOC 2, ISO 27001, ISO 13485) and BAA signatory status. HIPAA is not officially certified by any standards body, so we considered "HIPAA certified" a marketing claim unless supported by an audit report.
- Expert third-party verification: We evaluated verified reviews and third-party organizations, not the vendor's statement.
Why Security and Regulatory Compliance Are Non-Negotiable in Healthcare SaaS?
A Healthcare SaaS platform collects a lot of sensitive information, which includes patients' health records, medical history, insurance information, and payment information. Any security breach or non-compliance in the system may lead to exposure of this information and will result in significant penalties, lawsuits, regulatory actions, and loss of patient trust.
In order to regulate the use and handling of sensitive patient information, there are several laws, including HIPAA in the US and GDPR in the EU. Any violation of those regulations leads to heavy penalties. For instance, GDPR violation penalties could go up to €20 million or 4% of the company's annual global turnover, whichever is greater.
There are several necessary safety measures that should be included in healthcare business SaaS platforms, and these measures include:
- Data encryption for sensitive information
- Access controls for patient data
- Logs for tracking data access, modifications and any admin activity.
- Securing any data that would pass through secure integrations (EHR/EMR/other systems)
- Regular vulnerability assessment and penetration testing
All these safety requirements should be included from the very beginning of the software development cycle.

Also, the pricing and review figures above are directory snapshots and should be checked again before publication.
HITRUST, HIPAA and SOC 2: What Each One Means
Not all healthcare security badges are created equal. Some show that a company has passed an independent security assessment, while others represent the company's strategies for handling secure health data.
- HIPAA compliance: HIPAA is a US federal privacy law that addresses PHI, and not a certifying standard. A development company that manages PHI should be able to prove that they put appropriate measures in place around access, data encryption, and risk mitigation, and are comfortable with signing a Business Associate Agreement (BAA).
- The HITRUST Secure provides a standard, independently tested way of addressing safety and conformity controls. It comprises levels: e1 (a fundamental level), i1 (a medium level), and r2 (an extensive level). Instead of accepting a company's overall insurance claim that it is HITRUST, check how long the certification has been in place, what areas it covers, and whether it is still valid.
- SOC 2 compliance: SOC 2 is designed to verify controls related to security, availability, processing integrity, confidentiality, and privacy. A Type I report examines controls at a specific moment in time, while a Type II examines controls' effectiveness over a period of time.
A given development firm's certification doesn't necessarily translate to the healthcare SaaS product it creates. The product has to be subjected to its own security audits and compliance review(s) before launching.
An Average Cost of Healthcare Saas Platform Development
Rates vary by region and team model, and published numbers are limited. The rates we can confirm publicly are Suffescom's $25–$ 49 per hour and LeewayHertz's $50–$99 per hour. For the others, request a written estimate.
The biggest cost drivers are:
- Compliance scope (HIPAA only, or HITRUST and SOC 2 as well)
- EHR and FHIR/HL7 integrations
- Multi-tenant architecture and audit logging
- Penetration testing and security audits
- Post-launch maintenance and monitoring
Budget separately for audits, legal review of the BAA, and ongoing security testing. These are often left out of first quotes.
Questions to Ask Before You Sign
- Will you sign a BAA before we share any data?
- Which certifications does your company hold, and can I see the report or letter?
- How do you keep PHI out of development and staging environments?
- Have you built an Epic, Cerner, or other EHR integration? Can I speak to that client?
- Who owns the code and the cloud infrastructure at the end?
- When was your last penetration test or security audit, and can I see the summary?
- How do you log and audit access to PHI?
- What is your breach notification process, and how fast will you tell us?
- Will my team be your employees or subcontractors, and where are they located?
- Which versions of HL7 and FHIR have you worked with in production?
- What is not included in your estimate: audits, testing, maintenance, or third-party licenses?
- What happens to our data and system access when the contract ends?
FAQ
What is the difference between HITRUST e1, i1, and r2?
E1 represents the baseline and addresses all key security controls. I1 provides an intermediate level of assurance. R2 is the most rigorous and risk-based, and large hospitals and payers are increasingly requiring this from vendors that deal with high volumes of PHI.
Is SOC 2 enough, or do I need HITRUST too?
It's up to your buyers. SOC 2 is widely known and less expensive and quicker to achieve. HITRUST is more valued by healthcare buyers. Many companies begin with SOC 2 and then tier up to HITRUST when they begin to sell to bigger health systems.
Is my product HITRUST-certified if the vendor is HITRUST-certified?
No. The vendor certification applies to that vendor's company and the steps they are taking. However, your product will require its own assessment, although it may be less work for a vendor building to HITRUST controls.
What is a BAA? Do I need a BAA with a development company?
A Business Associate Agreement is an agreement that imposes a legal obligation on the vendor to safeguard PHI. You must have a business associate agreement with any vendor that might get access to PHI. Consult your attorney if a developer will only be working with de-identified or test data.
Can I use actual patient data for my tests?
They shouldn't. Use synthetic or anonymised data (if necessary), and ensure that only individuals named in auditing are granted access to production PHI.
What integrations really matter for healthcare SaaS?
Most products also require FHIR and HL7 integrations and connections to key EHRs, including Epic, Oracle Health (Cerner), and Athenahealth. Request a live demo of the particular system you require from each vendor.
What is multi-tenant architecture? And why does it matter in health?
This is multiple clients on one platform, with data separation. In healthcare, poor separation can mean one client's PHI is available to another — ask how the vendor keeps data separate, whether it is encrypted, and how access is logged.
How much does a HIPAA-compliant app cost?
It depends. Implementation compliance increases the work required for encryption, access controls, audit logs, security testing, and documentation. When you solicit proposals, request that they itemize these as separate line items to enable fair comparison.
Should I choose a fixed-price or time-and-materials contract?
Fixed price works for a clearly scoped product/ MVP. Time and materials is better for products that will evolve as you understand your users. A dedicated team is best for long-term platforms that require consistent development.
What red flags should I watch out for when selecting a healthcare development company?
- They refuse to sign a BAA.
- They "claim" they are "HIPAA certified" with no details.
- They have no healthcare references.
- They are uncooperative when asked how PHI is protected.
- Security testing and audits are not included in their estimate.
